Payment for Preservice Teaching Supervision

Thank you for agreeing to mentor a Preservice Teacher completing a period of Professional Experience as part of their study at Charles Darwin University (CDU).

For payment mentor teachers must complete and return :

• The payment information form - Contract of Supervision (Primary) or Contract of Supervision (Secondary)
  NOTE: The Preservice Teacher must sign the form to confirm the placement
• A Tax file number declaration form - available from school administration offices
• The Completed Overall Professional Experience Evaluation Record

At the completion of placement send all paperwork to:

It will take up to four weeks for payment to be made into the nominated bank account. All forms MUST be received by the Professional Experience Office within two months of the finishing date on the contract. Paperwork received after this date may not be processed.
